    Effects of Lift Velocity on Muscle Activation During Leg Extension

    It is not known if manipulating velocity within a prescribed resistance training mode will improve muscle activation. Muscle activations of the Rectus Femoris (RF), Vastus Lateralis (VL), Vastus Medialis (VM) and Bicep Femoris (BF) were examined during a leg extension exercise at 3 different velocities on 15 subjects (10men, 5 female, Age = 21.5 ± 1.8 yrs, Height = 171.2 ± 12.5 cm, Mass = 75.5 ± 16.3 kg). Trials of 1 set of 10 repetitions at 60% of 1RM, were performed at 15, 30 and 60Âș/s. Bipolar surface electrodes were placed over the BF, RF, VL, and VM. Micro-switches were utilized to identify the concentric (CON) and eccentric (ECC) phases of the lift. Data were sampled at 1024 Hz, filtered, rectified and the mean, integrated EMG calculated. One 2 x 4 x 3 (action x muscle x velocity) ANOVA with bonferonni adjustment was run and significance was followed by Tukey HSD post hoc analysis. Results indicated significantly greater activation of the VL, RF and VM for ECC extension at 60Âș/s compared to 15Âș/s. While 60Âș/s was also greater than 30Âș/s for the VL and VM during ECC. While comparing muscle action, CON VL, VM and RF were greater than ECC at 30Âș/sec, meanwhile VM CON was also greater at 15Âș/sec. No differences in muscle activation at any velocity or muscle action for BF were identified. We conclude that muscle recruitment while training with a 60% 1RM load is maximized at a velocity of 60Âș/s during ECC activity and 15 or 30Âș/sec during CON

    Contextual Insights into the Rett Syndrome Transcriptome

    Mutations in MECP2 are responsible for Rett syndrome (RTT), a severe X-linked neurological disorder characterized by loss of developmental milestones, intellectual disability and motor impairments. However, molecular insight into how these mutations affect the neuronal transcriptiome, disrupt neuronal function and contribute to RTT is impeded by the cellular heterogeneity of the mammalian brain. A comparison between gene expression changes in the striatum, hypothalamus, and cerebellum of MeCP2-null mice revealed that gene expression changes are distinct between different brain regions, which suggests that MeCP2 function should be understood in a cell type-dependent context. To accomplish this task, I generated and phenotypically characterized tagged knock-in mice bearing frequent RTT mutations in Mecp2 (T158M, R106W). Concurrently, I also developed a novel genetic system that allows the tag to be post-translationally modified with biotin in a cell type-specific manner. Altogether, these mice allow for molecular profiling of WT and mutant cortical neurons in the mammalian brain. MeCP2-dependent gene expression changes vary by age and cell type, and the degree of Pol II-mediated transcriptional changes genome-wide correlates with the severity of the RTT mutation. I also detected evidence that supports the post-transcriptional compensation of misregulated long genes, leading to a reinterpretation of prevailing thought within the RTT research field with regards to MeCP2 function. Finally, this genetic strategy circumvents genetic mosaicism associated with female mouse models of RTT and identifies functionally distinct transcriptional changes between neighboring WT and mutant neurons, therefore providing key insights into phenotypic severity between RTT-associated mutation types. By assessing RTT transcriptomes across various neuronal contexts, I propose a novel contextualized paradigm for MeCP2 function in neuronal cell types that not only hypothesizes how MeCP2 dysfunction leads to the cellular deficits that give rise to RTT-like phenotypes, but identifies novel transcriptional and post-transcriptional mechanisms that may be future therapeutic targets for RTT

    Treemaps: Visualizing Hierarchical and Categorical Data

    Treemaps are a graphical method for the visualization of hierarchical and categorical data sets. Treemap presentations of data shift mental workload from the cognitive to the perceptual systems, taking advantage of the human visual processing system to increase the bandwidth of the human-computer interface. Efficient use of display space allows for the simultaneous presentation of thousands of data records, as well as facilitating the presentation of semantic information. Treemaps let users see the forest and the trees by providing local detail in the context of a global overview, providing a visually engaging environment in which to analyze, search, explore and manipulate large data sets. The treemap method of hierarchical visualization, at its core, is based on the property of containment. This property of containment is a fundamental idea which powerfully encapsulates many of our reasons for constructing information hierarchies. All members of the treemap family of algorithms partition multi-dimensional display spaces based on weighted hierarchical data sets. In addition to generating treemaps and standard traditional hierarchical diagrams, the treemap algorithms extend non-hierarchical techniques such as bar and pie charts into the domain of hierarchical presentation. Treemap algorithms can be used to generate bar charts, outlines, traditional 2-D node and link diagrams, pie charts, cone trees, cam trees, drum trees, etc. Generating existing diagrams via treemap transformations is an exercise meant to show the power, ease, and generality with which alternative presentations can be generated from the basic treemap algorithms. Two controlled experiments with novice treemap users and real data highlight the strengths of treemaps. The first experiment with 12 subjects compares the Macintosh TreeVizTM implementation of treemaps with the UNIX command line for questions dealing with a 530 node file hierarchy. Treemaps are shown to significantly reduce user performance times for global file comparison tasks. A second experiment with 40 subjects compares treemaps with dynamic outlines for questions dealing with the allocation funds in the 1992 US Budget (357 node budget hierarchy). Treemap users are 50% faster overall and as much as 8 times faster for specific questions

    The state of the art of lethal oviposition trap-based mass tnterventions for arboviral control

    The intensifying expansion of arboviruses highlights the need for effective invasive Aedes control. While mass-trapping interventions have long been discredited as inefficient compared to insecticide applications, increasing levels of insecticide resistance, and the development of simple affordable traps that target and kill gravid female mosquitoes, show great promise. We summarize the methodologies and outcomes of recent lethal oviposition trap-based mass interventions for suppression of urban Aedes and their associated diseases. The evidence supports the recommendation of mass deployments of oviposition traps to suppress populations of invasive Aedes, although better measures of the effects on disease control are needed. Strategies associated with successful mass-trap deployments include: (1) high coverage (>80%) of the residential areas; (2) pre-intervention and/or parallel source reduction campaigns; (3) direct involvement of community members for economic long-term sustainability; and (4) use of new-generation larger traps (Autocidal Gravid Ovitrap, AGO; Gravid Aedes Trap, GAT) to outcompete remaining water-holding containers. While to the best of our knowledge all published studies so far have been on Ae. aegypti in resource-poor or tropical settings, we propose that mass deployment of lethal oviposition traps can be used for focused cost-effective control of temperate Ae. albopictus pre-empting arboviral epidemics and increasing participation of residents in urban mosquito control

    Evaluation of a Program to Improve Diabetes Care Through Intensified Care Management Activities and Diabetes Medication Copayment Reduction

    BACKGROUND: Medication copayment reduction can be integrated with disease management programs to incentivize patient engagement in chroniccare management. While disease management programs in diabetes havebeen evaluated across a range of settings and designs, less is knownregarding the effectiveness of copayment reduction as a component ofdisease management. OBJECTIVE: To evaluate the short-term results of a diabetes-focused disease management program that included copayment reduction, care coordination, and patient goal setting, focusing on rates of evidence-based care processes and all-cause pharmacy and health care costs. METHODS: Blue Cross & Blue Shield of Rhode Island offered large employer groups the opportunity to participate in a diabetes disease management initiative that featured reduced copayments (from 7/7/25/40forgeneric,tier2,andtier3drugs,respectively,to40 for generic, tier 2, and tier 3 drugs, respectively, to 0 for generic and 0− 0-2 for brand drugs) for diabetes-related medications. In return for the copayment reduction, participants agreed to the following: (a) participate in care coordination with a case manager, (b) have an annual physical examination, (c) have a hemoglobin A1c blood test at least twice annually, and (d) have a low-density lipoprotein cholesterol (LDL-C) test at least once annually. Patients received personalized support provided by a registered nurse and dietician, disease-related education provided by nurses, and intensified case management services, including working with a health coach to establish healthy behavioral change goals. All study subjects were aged 18 years or older and had at least 1 ICD-9-CM code for diabetes and at least 1 claim for an antidiabetic drug during a 12-month measurement period, which was each subject’s most recent 12-month period of continuous enrollment from January 1, 2008, through May 31, 2010. Administrative claims data were used to determine the percentage of intervention (participating) and nonintervention (nonparticipating) subjects from among all of the plan’s employer groups who received at least once-yearly monitoring of A1c, high-density lipoprotein cholesterol (HDL-C), and LDL-C; medical attention (or drug therapy) for nephropathy; and an eye examination. We conducted multivariate logistic regression analyses to assess the effect of the intervention and other patient characteristics and comorbidities on rates of performance of these care processes, aggregating the 5 processes of care into an “all or none” single composite outcome. We also developed a propensity score-weighted model to attempt to adjust for differences between the intervention and nonintervention groups resulting from the nonrandomized study design. Additionally, we quantified average plan payments to providers less patient copayments (i.e., net plan cost) per patient per year (PPPY) for the 12-month follow-up period and compared these costs for the intervention versus nonintervention groups. RESULTS: The study sample consisted of 9,698 patients with diabetes; 649 (6.7%) of whom participated in the intervention. 9,049 (93.3%) patients were identified by the insurer as patients with diabetes receiving usual care. Patients in the intervention and nonintervention groups were similarly likely to have all 5 recommended processes of care performed (40.1% vs. 38.9%, respectively, P = 0.543). Younger patients received all 5 recommended care processes less frequently than older patients (30.5%, 38.0%, and 47.0% for ages 18-48 years, 49-59 years, and 60 years or older, respectively, P \u3c 0.001); in adjusted analyses, patients aged 60 years or older were approximately twice as likely to receive all 5 care processes compared with patients aged 18-48 years (odds ratio [OR] = 1.97, 95% CI = 1.75-2.21). Users of oral antidiabetic monotherapy were least likely to have these processes of care performed compared with users of multiple oral therapies (OR = 1.23, 95% CI = 1.11-1.36) and insulin (OR = 1.59, 95% CI = 1.41-1.78). PPPY prescription drug costs incurred by the plan were greater for intervention than comparison patients (means [SDs] of 3,139[3,139 [3,426] vs. 2,854[2,854 [3,938], respectively, P \u3c 0.001); and the generic-dispensing ratio was slightly lower (means [SDs] of 62.1% [22.4%] and 65.4% [23.0%], respectively, P \u3c 0.001). There were no significant differences between the intervention and comparison groups in mean [SD] PPPY all-cause medical care costs (7,475[7,475 [17,601] vs. 8,577[ 8,577 [22,972], respectively, P = 0.213) or total all-cause costs (10,613[10,613 [18,590] vs. 11,431[11,431 [24,060], P = 0.666). CONCLUSIONS: Patients participating in this incentive program featuring diabetes medication copayment reduction and disease management components did not receive recommended care any more or less frequently than other enrolled members with diabetes. Younger patients and those utilizing oral antidiabetic monotherapy as their drug regimens were less likely to have the recommended processes of care performed. While prescription drug expenditures incurred by the plan were greater for intervention patients, between-group differences in total costs for medications and all-cause medical care were not statistically significant. Further follow-up is required to determine the success of this program over the longer term in promoting quality of care and achieving cost reductions and improved health outcomes

    Assessment of synthetic floral-based attractants and sugar baits to capture male and female Aedes aegypti (Diptera: Culicidae)

    Background: The viruses transmitted by Aedes aegypti, including dengue and Zika viruses, are rapidly expanding in geographic range and as a threat to public health. In response, control programs are increasingly turning to the use of sterile insect techniques resulting in a need to trap male Ae. aegypti to monitor the efficacy of the intervention. However, there is a lack of effective and cheap methods for trapping males. Thus, we attempted to exploit the physiological need to obtain energy from sugar feeding in order to passively capture male and female Ae. aegypti (nulliparous and gravid) in free-flight attraction assays. Candidate lures included previously identified floral-based (phenylacetaldehyde, linalool oxide, phenylethyl alcohol, and acetophenone) attractants and an attractive toxic sugar bait-based (ATSB) solution of guava and mango nectars. A free-flight attraction assay assessed the number of mosquitoes attracted to each candidate lure displayed individually. Then, a choice test was performed between the best-performing lure and a water control displayed in Gravid Aedes Traps (GAT). Results: Results from the attraction assays indicated that the ATSB solution of guava and mango nectars was the most promising lure candidate for males; unlike the floral-based attractants tested, it performed significantly better than the water control. Nulliparous and gravid females demonstrated no preference among the lures and water controls indicating a lack of attraction to floral-based attractants and sugar baits in a larger setting. Although the guava-mango ATSB lure was moderately attractive to males when presented directly (i.e. no need to enter a trap or other confinement), it failed to attract significantly more male, nulliparous female, or gravid female Ae. aegypti than water controls when presented inside a Gravid Aedes Trap. Conclusions: Our findings suggest that the use of volatile floral-based attractants and sugar mixtures that have been identified in the literature is not an effective lure by which to kill Ae. aegypti at ATSB stations nor capture them in the GAT. Future trapping efforts would likely be more successful if focused on more promising methods for capturing male and female Ae. aegypti
